Y8 HPV Immunisation Information

Polite reminder that the school vaccination team will be in school on Wednesday 12th Feb to administer the HPV vaccinations.

If you have a child in Year 8 and you have not already given your consent, this can be done on the following link using the unique school code: WF137655 https://www.wakefieldschoolimmunisations.co.uk/Forms/HPV

If you do not wish your child to receive the vaccination, please complete the ‘I do not consent’ section.

No pupils will be vaccinated without express permission from parents.
